IMMUNIZATION REQUIREMENTS
Code  504.1-E 
A T T E N T I O N    P A R E N T S    AND    GUARDIANS 
ALL STUDENTS
ENTERING THE LINN-MAR COMMUNITY SCHOOL DISTRICT
MUST MEET MINIMUM IMMUNIZATION REQUIREMENTS
BEFORE THEY WILL BE ALLOWED TO ATTEND SCHOOL.
All students are required by State Law to meet at least minimum immunization requirements before they
are allowed to attend school.  A record of these immunizations must be on file at the child's school.  No
student will be allowed to attend school without minimum immunizations. 
MINIMUM IMMUNIZATIONS
REQUIRED TO ENROLL IN SCHOOL:
1.
One DPT.
2.
One Polio.
3.
One Measles/Rubella given after the age
of 12 months.
     4.    One dose of Hepatitis B if born
on or after July 1, 1994.
5.
One dose of Varicella vaccine given after
the age of 12 months if born on or after
September 15, 1997, or reliable history of
the disease.
 
    SUMMARY OF REQUIREMENTS OF
THE IOWA IMMUNIZATION LAW:
1.
Four DPT if born on or after September 15,
2001, three DPT if born before September
15, 2001 (last dose after the age of four).
2.
Three Polio (last dose after the age of four).
3.  
Two doses of Measles/Rubella if enrolling
after July 3, 1991.  The first dose must be
given after the age of 12 months.  The second 
dose must be separated from the first by 
no less than 28 days. 
4.
Three doses of Hepatitis B vaccine 
if born on or after July 1, 1994.
5.
One dose of Varicella vaccine given after 
the age of 12 months if born on or after
September 15, 1997, or reliable history of 
the disease.
If your child has begun immunizations but has not yet received all the required immunizations, he or she may be able to
attend school on a provisional or conditional basis.  To qualify for provisional enrollment, your child must have received
at least one dose of each of the required immunizations.  If your child falls into this category, you must submit an Iowa
Department of Public Health Provisional Certificate of Enrollment to the school.  This “certificate” will expire in 60
days, so you must have your child completely immunized during this time or he or she will not be permitted to attend
school until the immunizations have been received.  When your child has received all the required immunizations, you
must submit a completed Certificate of Immunization to the school.
